Amendment Letter Submit an amendment letter to request changes to a study which already has research ethics approval. Attach any new documents for review, or any documents which are changing. Highlight or mark the changes for ease of review. Ensure the version number and date of any amended documents is updated. Version numbers are usually located on the first page of the ethics application, and often in the footers of the information leaflets and consent forms. |

Data Protection / DPIA Review Review your DPIA when making any amendment to the ethics application |

Annual Progress Report Submit a progress report at least annually for all active studies. Please delegate submission of annual progress reports to a member of your research team. Where annual reports are not received, the study will be listed as 'lost to contact' and research ethics committee approval will lapse. | |
